Pittsburg Community Schools in partnership with CHC-SEK is providing sports physicals to all students starting May 1.

 

Any sports physical done after May 1 is valid for the entire upcoming school year.

 

There will be no out-of-pocket cost for the sports physical.

CHC-SEK will bill your child’s insurance if applicable, but will not bill individual families for providing this service. If you would like your child to participate, please submit the completed paperwork to your school nurse.

 

Sports physical packets are available to pick up in the nurse clinic at PHS, PCMS, and all elementary schools. Once parents complete their section of the packet it can be returned to the school nurse prior to May 1.
